THE LOWER OUTAOUAIS FAMILY HEALTH TEAM (LOFHT)!

Family health teams (FHT) are part of the Ontario health care system and are designed to support family doctors by providing additional services from other health care providers.

As the team is funded by the province, we can only accept referrals for Ontario residents who are enrolled with one of our family physicians. Your doctor will have a specific reason for sending you here to be seen by one of our interdisciplinary health professionals (IHPs) including nurse practitioners, registered nurses, mental health counselors, dietitian, pharmacist and health promoter.
